How to Apply for the NDIS

Step-by-Step Guide

1. Check eligibility by visiting ndis.gov.au or call 1800 800 110.
2. Request an Access Request Form (ARF) or apply online.
3. Submit your ARF with supporting evidence from your GP, specialists, or therapists.
4. Undergo an NDIA assessment – this may involve interviews and documentation reviews.
5. Attend a planning meeting to create your first NDIS plan.
6. Nominate Level Up Care Services to deliver your funded supports.
7. Services can begin as soon as your plan is approved.
