How to Apply for a Home Care Package (Aged Care)

If you or your loved one is aged 65+ (or 50+ for Aboriginal and Torres Strait Islander
people) and needs extra support to live at home, a Home Care Package can provide
government-funded assistance tailored to your needs.

Step-by-Step Guide

1. Call My Aged Care on 1800 200 422 or apply online at myagedcare.gov.au
2. Book an in-home assessment with an Aged Care Assessment Team (ACAT).
3. Receive a letter with your approved Home Care Package level (Level 1–8).
4. Join the national waitlist – this may take several weeks to months depending on priority.
5. Once assigned a package, choose Level Up Care Services as your provider.
6. We’ll meet with you, design a care plan, and activate your funding for immediate service
delivery.

Helpful Tips

– Keep your Medicare card ready when calling.
– You can nominate a family member or friend to speak on your behalf.
– You may receive an interim package while waiting for a higher level.
