Navigating Your Aged Care Journey
1. Apply Through Level Up Care
Call My Aged Care on 1800 200 422 or apply online at myagedcare.gov.au
2. Assessment and Eligibility
Book an in-home assessment with an Aged Care Assessment Team (ACAT). Receive a letter with your approved Home Care Package level (Level 1–8).
3. Developing Your Personalised Care Plan
Join the national waitlist – this may take several weeks to months depending on priority. Once assigned a package, choose Level Up Care Services as your provider.
4. Commencing Services
We’ll meet with you, design a care plan, and activate your funding for immediate service delivery.

Costs and Funding
Home Care Packages are subsidised by the Australian Government.
Depending on your income, you may be required to contribute:
Basic Daily Fee: A standard fee that some providers may charge
Income-Tested Care Fee: An additional contribution determined by Services Australia based on your income assessment.
